What do I need to get my driving record?

To obtain your driving record, you must submit certain information. You’ll need to fill out basic information like your full name and current address, as well as contact information like your phone number and email address. You’ll also need to list your date of birth and the last four digits of your social security number. Finally, you’ll need to fill out information from your Texas driver’s license, including your driver’s license number and the audit number (DD) listed on the bottom of your license.

What documents are required to get a birth certificate replacement?

Texas requires basic information about the person requesting the certificate, such as your full name and address, plus contact information such as your email address and phone number. You’ll also need to list what your relationship is with the person on the birth certificate and your reasons for requesting it if you are replacing it for someone other than yourself. You can generally only obtain your own birth certificate or a close family member’s birth certificate, like your child, parent, sibling, or grandparent.

Next, you must fill out information pertaining to the person whose birth certificate you are trying to obtain and the circumstances of their birth. You’ll have to list their name, date of birth, place of birth, and parents’ names.

In addition to the information listed above, you must submit one of the following items, along with your application form, to satisfy the requirement for certified Texas birth certificates:

  • Federal or Texas state ID card
  • US passport
  • US driver’s license

Who can apply for a new or replacement birth certificate?

To apply for a birth certificate as a self-applicant, you must be at least 18 years old. If you want to obtain a birth certificate for your child, you must select “parent” or “guardian” during the registration process. You can also request a birth certificate if you are a close relative like a sibling or grandparent.

How long does it take to get a copy of your birth certificate in Texas?

TX-DPS offers standard shipping after your request has been processed. For an extra charge, expedited delivery is available for birth and death certificates and will be processed within 3 days of your request.
